How do I submit a Health History?

Health Histories will be submitted through the Health History Timeline of your Learning Center, which will open at Module 10.
Please note that the intake forms that the mock client fills out will not be uploaded into the Learning Center. After you have conducted your session, please complete the series of short answer questions for each of the six required Health History consultations.
